Hi everyone.

I run a website and I work in real estate in Miami, Florida. I would like to have some video interview as well as some non-interview related videos.

What forms do I need for the interview itself, for the individual to be quoted, to have the video as my personal property and to be allowed to use it on youtube or my own website, and to use the likeness of the individual in the video, and so that no other company or individual can claim that they have a PR document signed by the individual that supersedes anything that I have done with them so that I don't have to take down the video, and also to use my website logo embedded on to the video, and I want the ability to maybe use the videos I take in maybe a movie/documentary/show or something to that affect. I just want to have that ability.

I figure I need a model release form, an interview form, and is that it? How much do things like that run, and is this something that I can create myself?

You can locate sample forms online and then tailor them to your needs. It is always wise to have an attorney review your consent and release forms prior to use, however, to make sure you have yourself fully covered.

They all have important information regarding permissions needed, and why permission is needed, and they all have forms you can use to help protect your videos, interviews, and photography from lawsuits that could otherwise result from the publication of the material on your website or elsewhere.
