Q. can they force me to send a certen amount every month.

A. They can't "force" anything, until they sue you, get a judgment and start taking assets, instead of waiting for whatever voluntary payment you're going to make. You may be lucky that they've waiting this long and, if you've been paying by check, they now have current asset information (and probably a whole lot more from your hospital admission information).

Just a suggestion, but try to make a regular plan that both sides can live with and you can perform before they just get itred of waiting. If that happens, the eventual costs to you will be much greater.

As to the title on your post, from their standpoint, you're the loan shark. Unless you had this agreement is advance (which I sriously doubt), they rendered services with the expectation of being piad in full. You're paying in installments that change regularly. Now if they billed you in monthly installments but said "this month the installment is $X and a late payment will earn interest at Y%" and those figures changed monthly, people could hear you screaming from OK. That's what you're doing to them.
