Form to re-schedule a hearing - CMC

What is the name of your state (only U.S. law)? California

i need to know which form that i have to use to re-schedule a hearing for a CMC**************please help!!

Are you referring to a mediation conference ?

Call them as soon as possible and advise them as to why you need to be rescheduled. Follow their instructions about advising the other parent of a rescheduled date (they should give notice of a new date, but check and make sure they will).
