I need HELP!

What is the name of your state?CA

\I am filing for an ex parte restraining order and order to show cause re preliminary injuction in Los Angeles against a consumer reporting agency. I can not find out exactly what papers I need to file. I have to get this done today to file first thing in the morning...Thanks in advance!
