If an insurance claim check was made payble to myself "and" my ex wife, and she deposited this check into her personal checking account without my signature, what rights do I have to obtain this money? What are my options and who do I contact - The bank, the insurance company, or my ex?

You have the 'right' to ask her for it.

Depends. Was your name on the check as an endorsee?? If so, who signed your name??
