Replying to summons (form name or number)

What is the name of your state? CA
When replying to a summons. Is there a specific form one needs to fill out and if so can it be downloaded from a government or other site so I don't have to make two trips to the court house ? A form name, number or both will be greatly appreciated. Thanks
