Trial Date Stipulation and Agreement for Continuance

What is the name of your state (only U.S. law)? CA

I have a trial date set for a lawsuit stemming from a debt collection. I am hoping to get a continuance but I understand I need a stipulation agreement signed by all parties. I am representing myself. Is there a stipulation agreement form I can use?
