45 days have passed no notification on claim for LTD

What is the name of your state (only U.S. law)? CA

Hello,

I filled my claim on 10/26/10 for Long Term Disability, spoke to case manager on 11/12/10. Have not be given any notice, if approved or if they need more time to decide. From my understanding they need to inform me within 45 days. What would be my next step ?
