What is the name of your state (only U.S. law)? PA Need help. Had foot surgery on Aug 24. On STD and FMLA. Employer set ending date for STD at 11/21. Now have back surgery scheduled for Nov 26. Employer stated that I will be terminated when I go on LTD. When I file for LTD what should I use for disability date?
