What is the name of your state? CA (Victorville, San Bernardino County)

I just recieved my divorce default packet back from the courthouse apparently, some documents were missing and, some incomplete. I pretty much figured out most of what I need except the following - maybe I could get a little input?

1) Need Declaration Re Service of Dec of Disclosure (Preliminary)
2) Noted on my return slip: "To file the stip to establish or modify child support with respondents signature requires him to pay fee's of $303.20. It is not needed with your Default packet." (What does that mean?)

My husband and I have a written agreement for the issues of the divorce including child support, visitation, etc... I have attached this document to the form(s) where is required.
