az divorce w/o children question re: other cases on form drfn10fn

What is the name of your state? arizona
filing for divorce without minor children there is a question on form drfn10f that states "have there been any other cases (excluding minor traffic offenses) in any court involving members of this family?" petitioner has spent time in the past in prison. does this question require information regarding these charges including the case numbers pertaining to these court involvements with pertaining case numbers and if so, how far back is such information required?
