confuzed with paperwork

I am currently doing a divorce myself and i need some help legal aid sucks out here and I am doing a true default packet, I have questions on a few of the forms am I suppose to fill them out or is this the judgment ??? I have no clue and everyone keeps giving me the run-around does any one know who fills out forms FL-180, FL-341, do i fill these out or is this a judge that fills these out im clueless!!!! PLEASSE SOMEONE HELP ME!!!
