I was married by common law in 1982, I lived with my common law husband for 12 years, I had legally taken his last name. I moved to Alaska in 1994. My qustion is, what steps do I have to take to take my last name back?

Get divorced and ask for your name to revert to your maiden name as part of the divorce.
