Division of 401K assets

What is the name of your state? Arizona

My wife of 5 years and I are going to divorce. I have over 100K in my 401K which is half hers. I'm told that a QRDO or Qualified Relations Domestic Order will divide the retirement plan to create a new plan in her name and avoid taxes and penalties. Is this true? Anyone with experience regarding this situation? Thanks.
