What is the name of your state (only U.S. law)? California

Hi I just have a quick question, when filing a regular divorce, do I have to fill out form 160, the financial disclosure that lists my assets/debts?

I am a young guy, 26 years old, nothing is being split up or contested. The worker at the Burbank court gave me a packet without form 160, however when I went to the self-help center the packet they told me I need includes the 160 and all the financial stuff.

My dad got divorced short while ago and I don't remember seeing the form 160 in his paperwork
