What is the name of your state? Maine

My current wife's X is not complying with the divorce judgement in that he won't pay a certain percentage of her kids medical bills. We want to sue him to comply. Neither of them live in the state where they got divorced. He lives in the mid west somewhere and my wife lives in Maine. Where do we sue him?

Why sue? He is in contempt of a Court Order. Contact the Court.
That still requires a lawsuit called a Show Cause.
OP, it needs to be filed in the court that has jurisdiction. if the children reside with your wife and neither parent resides in the state where the order is then she needs to domesticate the order in maine provided she has lived there long enough to establish residency. Then once she domesticates the order THEN she needs to file the Show Cause.

Ok, after she domesticates the order in Maine and then files Show Cause how is he notifiied of this in his home state? How would we or the local court enforce the order?

He's properly served according to the local rules of the new court.

"WE" don't do anything. This is her deal.
