divorced two yrs.-the x has medical insurance now-i want to drop her from my plan how

What is the name of your state? Rhode island
I have been divorce for two years. The divorce papers have her on my medical plan right now. She did not have medical insurance two years ago. She went full time on her job and now has medical insurance on her own plan. What do I have to do to get her off my plan? I think I need something in writing to have in case she trys to claim she is on my medical insurance plan. Note: she never did use my plan as of yet that I know of. Pete
