I can't find anywhere a clear answer about how to "amend" FL-115. I filed FL-115 second time because I had to serve amended petition and my FL-115 was rejected because :"Your proof of service does not indicate service of the Amended Petition as required" . I don't know where exactly I should write or mark on the FL-115 that the amended petition was served.
The rejection paper says: "A newly executed, new original FL-115 form is required." Does that mean the second FL-115 and marked somewhere that the amended petition was served or a completely new FL-115 needs to be submitted?
