How do I file contempt charges?

What is the name of your state (only U.S. law)? nebraska
My Ex has continually not followed the divorce order in several cases.
1. I was given the tax deduction for our youngest child, and will not sign the papers that the IRS Requires so I can claim him.

2. She was ordered to return items and has not and refused to do so.

What is the format of the papers I need to submit so I can take her to court and force her to sign the papers and return my items?What is the name of your state (only U.S. law)?
