What is the name of your state (only U.S. law)? CA

I have received a request from my wife for additional information with respect to some insurance items. Since she has an attorney, does this request not have to come from her lawyer? Also is there a request form. I guess we are in the discovery phase.

If she has retained an attorney, the request should come from the attorney, but what difference does it make? You're eventually going to have to provide the information, anyway.
