please help (divorcing 101)

i have three questions... please be patient with me i'm so confused.

1. if my husband and i agree on divorce, material possessions divided to agreement, no children, no property... can we both sign the divorce papers before handing them to the court system (no serving and such)?

2. also, are we required to go to court in this case?

3. if i married and changed my name on the marriage documents but never recieved a new social security card with my new last name, are the divorce papers to be filed with my maiden name or the name i recieved at marriage? also, my taxes this year were filed with my maiden name because my last name was never changed at my workplace either.

if anyone could help me that would be great. i think this site is very helpful.
