I need to file a petition please help.

What is the name of your state? PA.

I need to file a petion for my boyfriend in jail. He got a 60 day sentence for driving under a DUI suspension. He has served 30 days and wants to do the last 30 under house arrest or intensive probation. In order to do that I need to file a petition. I need to know the correct way to write one. Can any one help?
