Reinstate driver's license

What is the name of your state (only U.S. law)? AZ
I have the 'investigation package'. What I am trying to find are AZ MVD 'approved' substance abuse counseling services. There a few listed, but trying to find anything online seems to be a near impossible task. There has to be more than the four on this list.
