Hello, I am currently on the process of filling out an AD and since the form I received gives me this itty bitty box for the special instructions section, I am going to need to add additional pages to this will. What I need help on is when you add additional pages to the living will, do you need to put certain things on it, similar to a statement I found in a book on living wills:

I____________ hereby execute this addenum to the living will, executed by me on _________, 200_. The sole purpose of this addenum is to more fully express my wishes regarding my medical treatment. If any or all of the terms of this addenum are deemed to be invalid, my living will shall remain in effect.

and also should each additional page be signed by the person writing the living will and his or her witnesses?
