My mother wants me to transfer some real-estate from her name to my two brothers and my name.My mother is homebound. Since I am listed as having her durable power-of-attoney,can I take care of this matter without her having to be present?

If you have a durable power of attorney for property/finances (there's also a durable POA for health care) & meets the state's requirements for a legally enforceable POA & if your Mother gave you broad powers (including transferring of real-estate), then, yes, you can take care of the matter yourself.
