Friend in nursing home

What is the name of your state (only U.S. law)? PA
I have an 83 year old friend who is 17 years my elder, and her sisters who are close in age and not well have asked me to take care of their sister who is now in skilled nursing. She has needed nursing home care twice before this year, and her 100 days will soon be up I feel sure. I have no idea how to start helping her apply for Medicaid (Medical Assistance) or what I need to do. I do have POA for health care for her. She signed herself into the nursing home from a hospital. She has Parkinson's Disease and a fractured hip and some degree of dementia. She is able to sign her name and generally is considered competent, but she is confused at times especially lately. I would appreciate help on what to do.

Talk with the social worker at the facility.
