What is the name of your state (only U.S. law)? Texas

My grandmother was assigned a guardian ad litem without my knowledge. I have a previously written medical power of attorney. Do I have the write to make decisions on her behalf without consulting with the guardian ad litem?

Without reading the court order, no one can tell you. Go to the court house to read the document or try calling the court house to have someone read you the document appointing granny guardian ad litem.
