health care proxy- translation?

NY.
How can a person who does not speak english excecute a health care proxy? Would the health care proxy need to be in that persons language and then translated into english inorder for doctors to view it?

If the document is not in English, how do you suppose health care providers, including physicians, would be able to read and understand it?
