I live Indiana. My baby is nine months old. when he was born i gave him his father's middle and last name. (his father promised we'd be married and he would take care of him, blah, blah, blah) it never happened.I am going to change my son's middle and last name. i would like to know how i should go about in doing so.

Why do you want to change it? Is it out of spite b/c he didn't marry you? You named your son after his father b/c you were in love and your son is a result of the love you shared so why go and change his name?
