Question


I am a single mother, and my daughter currently has her father's last name. We were never married, he does not pay child support, he does not see our daughter and he rarely calls her. I would like to know how I can go about changing her last name to mine. I live in Illinois. Thank you in advance for your help.

Go to the courthouse and pick up the papers. Have a legal aid or paralegal help you fill out the paperwork. You will have to publish in his local paper or notify your ex in some way that this is happening. If he doesn't contest it, you're all set. You'll have a quick hearing before the judge and it's a done deal.
