Minor name change

I live in North Carolina. My son was born in VA. Birth certificate has last name of biological father. He uses my last name, also has a social security card with my last name. Also has card with biological fathers last name. Biological father has never been in my sons life, pays no support, never made an attempt to see child. Can I just aply for a name change? Have no idea of where his biological father is. My son has stated he doesn't want his name for his last name. He is 11. Can he petition for his name to be changed himself. Please help me. Really confussed.
