modifying child cust. & support

I live in Texas, my children(they are 14 and 12), want to come live with me and this is agreeable with their mother.
Is a lawyer nessassary?
Without getting a lawyer, how can I modify the original div. decree and where would I
go to get the correct forms?
Is there a pre printed form where I can just fill in the blanks, and just get her to sign and notorize it?
I'm not askink for support from her, but want mine to stop.
