Name Change(minor)

When my daughter was born,I gave her her fathers last name. We are no longer together and now I want to add my last name to her existing name.What do I have to do?

My son just had his whole name changed. He was named for my x who disappeared when he was 3 months old. He has had no contact with him in 12 years or paid child support. All we had to do was to file a petition with the court, a few changes of name in the paper, wait for the approriate amount of time and go to court. You should contact an atourney in your state to see what all you need. Good Luck. I shold also mention it cost around $500 for this.
