Parental rights terminated??

As a teenager I Fathered a child. The Grandparents thought it was in the child's best interest to adopt this child. We went to court, I gave up all parental rights, Grandparents received full custody and were to start adoption proceedings. This same Court Order restrained me from going within 300 yards of the child. Just this week, I was served papers from the State of Wisconsin, I live in Georgia, seeking child support. It seems that they Grandparents never adopted him and my rights were not terminated. And it only took the State of Wisconsin "12" years to inform me of this!!! I'm at a complete loss as what to do. I've never heard of this kind of thing happening to anyone else. HELP????
I'd be very grateful for any advice.
