Uncontested custody transfer

My 16 y/o stepson has decided that he wants to come live with his dad. His mother has agreed. Now what do we do? Do we need to do this with lawyers and a judge or is there some paperwork that we can do ourselves. We are on a very tight schedule as he says living there is unbearable and wants my husband to pick him up next weekend. We need some sort of custody papers to register him in school. Is there a way to do this fast? and inexpensive!?
