FMLA help, provider not completing form correctly

I am eligible for FMLA leave for the care of my daughter.
Our company outsources this to a TPA.
I completed the forms and sent them to the provider who
Filled them out and returned them, but the TPA says the dr
Was not specific enough in regards to frequency of treatments
And duration of condition. So, I contacted the provider, sent
The forms again and have been ignored. The provider will not
Return calls to me or the TPA acct manager and my leave has
Been denied for the forms not being completed timely. I now
Have to start a new case . Is there a way to compel the provider
To do the forms completely and timely? I have accrued absences
That would be coveted under this leave, had they done the forms
Correctly!

With the provider? No, I haven't. It is an
Outpatient treatment center 2 hours away.
Isn't there some federal verbiage that would
Force them to do the forms properly and not
Just do a "general" sign off?

If you want something done in a specific way, you need to go there and discuss with them exactly what needs to be included. No one likes to work for free - don't be surprised if there is a fee to complete the form.

Properly according to the TPA..... I had FMLA years ago and it was much easier...Cbg,
I suspect we have worked for the same company (I lurk).
This TPA is insanely particular about the forms and it is tough
To get approved. My good friend has a son with a chronic lifelong
Condition and they require her to re certify ever 3 months.

I would gladly pay a fee if they would complete
The forms do I don't get any more occurances.

What's irking me is being in the middle...this is an outpatient
Treatment program, no set end date. If my daughter's rides
Get cancelled she cannot drive after treatment so I need
To take time off to get her there and back.

I don't want to lose my job because this sketchy treatment
Center refuses to comply with the TPA or do the forms to their
"Satisfaction".
