Braces Dental Treatment advise?

Hi, I having a braces treatment for 24 months ending in October 2008, i have HMO Dental Insurance, so, I just realize with the insurance that i suppose to pay US1900 orthodontic treatment +250 of retencion , but the dental office is charging me US1900 +( 500 +500)=2900, the description for 500 charges was Minor TX, H Habits, Fixed Appl,

My question is what means in english the above description because they charge me two times the same (500)

I will really appreciate if some body who know about it, or advise what to do with them.What is the name of your state (only U.S. law)?

that sound like a harmful habit appliance. did they give that to you for grinding your teeth? or are they considering it as part of the orthodontic treatment? you should also ask your insurance company if they are covering that appliance or not**************..
