Florida

Is my Dr. obligated to file a claim to my insurance for a flu injection if he is a provider of my insurance?What is the name of your state (only U.S. law)?Florida

Unless your insurance policy SPECIFICALLY says that it is the responsibility of the doctor to submit claims, no, he is not so obligated.

Normally contracted physicians ARE required to submit to your insurance before billing you. Are flu shots covered on your plan?
