Health INsurance problemWhat is the name of your state (only U.S. law)? NJ My Daughter is receiving ongoing treatment,that she has paid for out of pocket, We submitted the Bills to A Federal Insurance Health Plan. Insurance Company claims that when they entered Facilities Federal ID # they received no information, and have requested Dr.s Social Security # and copy of license. Facility will not give us this info, Insurance wont process without it. Any advice on how to proceed? Thanks |

| Facility needs to give your insurance company the info - YOU don't need to have it, but it is public information (TIN anyway, and SS if they are using it as TIN) and if they are unwilling to provide it to the insurance company, red flags go off as to the legitimacy of the provider.
