Precertification

What is the name of your state? TX

My husband's company (Reno, NV) decided to close Texas branch. Told him that he would be laid off effective Feb. 28, 2006 (in writing). I went in for Pre-op on Feb. 22, 2006. All insurance coverage confirmed for surgery scheduled for Feb. 24, 2006.

On Feb. 22, a letter was sent via FEDEX stating my husband was being dismissed effective Feb. 23, 2006. We received on Feb. 23.

Should the surgery, which was performed on Feb. 24, be covered under original plan? Or will Cobra be required to cover?

Note: Company self-insured and experiencing financial difficulty. Husband caught in crossfire between former CEO (son) and his father.

When does your policy terminate at the end of the month or on the day of termination (*Hint* Check your benefit booklet or call Customer service)?

If your policy ended before the surgery it will not be covered and you will want to take out cobra coverage if the bills are larger then the monthly cobra premium payment.

Pre-authorization is based on benefits available at the time of service (I.E no eligibility no benefits available).
