I am a licensed acupuncturist in the state of Minnesota and would like to receive referrals from another acupuncturist. These referrals would require me to use her clinic space when she is not there, so that I may bill insurance. The person making the referrals would perform insurance billing for me and require a percentage/fee from each client. I do not want to be construed as an employee-is there a contract or other legal document I could use to establish my status as an independent contractor?

You can execute a contract to reimburse her for your use of space, supplies, and billing service. It's done all the time.
