My employer notified short-term disability carrier that my employment was terminated prior to my filing for short-term disability benefits. Employer knew that I was on unpaid time-off between last day worked and doctor's notification of disability.

You'll have to prove that and it should not be hard. A suit against the insurer and employer may be needed.
