My sister was hired by a small company in Texas during her OPT (optional practical training) and her H-1B was granted soon after that. Recently her H-1B was renewed after 3 years of working. She has all the document to show that she is legally staying and working in the US.
However, since she has never left the US in the past 4 years, her foreign passport still has no record of the H-1B VISA. (What in that passport is her already expired F-1 VISA!) Will she get into any trouble if she travels abroad and re-enters the US?

What your sister has now is H-1B --status-- which is fine for exactly what she has been doing.
However, if she wants to travel, she needs a travel document, a visa sticker, so that when she returns, she can be re-admitted in that status.
She will need to make a Consular appointment and get her visa sticker while she is abroad. She can not return to the US with the documents she has now.
