What do I legally have to disclose to child's high school?

My child's high school sent home participation forms to be completed today. Along with asking about general information, there's a question regarding juvenile convictions. Is this a question they're allowed to ask? If so, how much information do I disclose and how can they request information that's supposedly 'sealed'?
