What is the name of your state? Illinois

I need to know the exact procedures to evict my significant other. I filed with the court in October 2004, but because of wrong information given to me by the county clerk's office I had several continuances. I missed my last court date because of illiness. I now have to motion up again and I want to make sure it's done right this time. I don't want any money from him, I just want him to move out of my house.

Do I have to send him a 30 day notice by ceritified mail?
Does the 30 day notice have to be received by him before the 1st of any given month with the move out date the end of the month?
