Eviction for a lodger - SUM-130 - "Individual defendant" or "Occupant" or BOTH

What is the name of your state (only U.S. law)? CALIFORNIA - Los Angeles

Hi all, I will file an eviction against someone renting room in my house because he did not move out after the expiration of the "30 days notice to quit".
In the Summons form (SUM-130) that I need to file to the court, I don't know what to check on the box for this question#4

4. NOTICE TO THE PERSON SERVED: You are served
a. as an individual defendant
b. as the person sued under the fictitious name of (specify):
c. as an occupant

Should I check the box for "individual defendant"? "occupant"? Or both.
Please help. Thank you so much.
