Is my lease still in effect?..Please help.

my fiance and i have been having issues with our landlord/contracting company. First of all, when we looked at this apartment, a lady who called herself the "rental agent" said she would be taking care of all the lease signing and all that. well she did, but she as very "shady". When we gave her our security deposit (mind you this was all done at the door of her run down truck, this landlord owns 4 complexes but has no office to speak of) she didn't have the right forms so she scribbled out words on the last months rent form and wrote in security deposit. she also contacted us 2 weeks after we gave her the security deposit to fill out another form for it to be put into an account (obviously not very organized). when she returned 2 weeks later for us to sign the lease we told her that there were some problems in the apartment that needed to be fixed (kitchen drawer tracks broken, exposed wires in the living room, 2 burners on the gas stover needed a lighter to light then, and ripped up tack strip on the carpeting entering the bedroom). she just shrugged it off and to this day only the carpet has been fixed. we have made several attempts to have these things fixed. that is problem number 1. number 2 is this, we never recieved a letter stating the bank name, adress and account numbers where our security deposit is. when i looked up the laws it states that within 30 days tenants are to recieve a letter stating those things, and we havent..we have been in this apartment since april 1.
we just need to know if because they violated the law, if our lease is now void and what you all think we should do.....help!
