Questions on how to fill out CT Civil Summons Form

What is the name of your state (only U.S. law)? NH

1. What box do I check to file against an auto dealership: Judicial Distr., Housing Session, or G.A.#??
2. Case Type Codes from page 2 V06 and V09 sound like the correct major and minor choices for buying a truck with a major motor defect from a dealership?
3. Do I arbitrarily select any return date, as long as it's a Tuesday?
4. Who would be the person(s) I place at the bottom where a name and address of the person to prosecute in the amount of $250?

Why can't they make these forms easier to understand?

Thanks for the detailed reply. I contacted the DOT since it was a dealership, but they waited nearly a year to act and now the dealership is no longer franchised.
My next move is through the courts since they have a bond filed for cases.
