Adding father's name to birth certificate

Hello, I just need to know what procedures I need to take in order to get my name as the father on my 6 year old biological son's birth certificate. For some reason the paperwork was never filled out at the time of his birth. Both his Mother and I are in agreement that my name needs to be added. One thing to note is that we were never married. I never had noticed that my name was not on it until I went to get him a passport. Othewise, I would have obviously done this sooner. I have had legal visitation and have paid child support his whole life. Thanks.

The only thing I had to do was contact the vital records department and request the form to fill out. It was a simple form. Depending on the state's laws, you can either be added by voluntarily acknowledging paternity or by getting a paternity test done.
