What is the name of your state (only U.S. law)? KY

I have a problem with my son's birth certificate, and never noticed it until renewing information for insurance.
I was wondering the best way to get my name added as his father, yes I am the biological father and yes I did sign at the hospital for it to be on there, however, it is not on the one we recieved in the mail. What is the best/quickest way to get my name put back on as the father so that I can continue to insure my son?
